Probable Migraine Versus Migraine: For Concept to be on the Continuum

Abstract
Background: A considerable proportion of individuals suffering from headache were classified as probable migraine, which is fulfilling all but one criterion for migraine. In the continuum severity theory, the mild end is thought to
be tension type headache which transform into migraine with increasing severity of condition and the gray zone of
headache occupies the middle ground of the continuum. Because of dichotomous diagnostic criteria between migraine
and tension type headache, the position of probable migraine also could be located at this gray zone.

Objective: The present study aimed to evaluate whether the patients with probable migraine might be part of migraine or representative for heterogenous group between migraine and tension type headache.

Methods: The present study is a nation-wide cross-sectional descriptive survey with proportional to population distribution that was conducted using a structured interview using questionnaire to determine the status of headache disorders by trained interviewers. The subject with probable migraine (PM), strict migraine (SM), and tension type headache (TTH) were identified according to the ICHD-II. We evaluated sociodemographic profiles and the impact on headache related disability of the individuals with PM compared to those with SM and TTH.

Results: Among 1,507 individuals, 109 (7.2%) individuals with PM, 90 (5.9%) with SM and 464 (30.8%) with TTH were identified. The female predominant prevalence was prominent in PM (76.1%), which was similar to those of SM
(75.6%) but contrast to those of TTH (43.8%). The headache pain intensity, pattern of analgesics use of individuals with PM were similar to those of SM but they were significantly different from those with TTH (p<0.01). With regard to headache related disability, total score of HIT-6 and, the proportion of grade in HIT-6 were similar in both PM and SM, they are comparable to those of TTH. The difference between PM and TTH was statistically significant (p<0.01).

Conclusion: The present study supports the concept that probable migraine might be a continum of migraine. Probable migraine has considerable amount of disability as migraine.
